Under the Health Records Act 2001 and the Privacy Amendment (Private Sector) Act 2000 patients have the right to access their health information held by Epworth HealthCare.
Patients can request either a copy of their medical record, an accurate summary or to view their medical record.
Epworth HealthCare requires payment of fees for access to patient information as per the Health Records Act 2001. See Medical Record Access Fees download below.
Requests can be made by completing the Medical Record Access Form (download below) and returning this with a copy of photo identification to the Privacy Officer, Health Information Services at the site that information is required from.
