Patient Admissions at Epworth Eastern

To help us prepare for your admission we require your admission details prior to the day of your admission. This form should be sent as soon as possible to our Patient Administration Office at 1 Arnold Street, Box Hill, 3128 or faxed to 03 8807 7676.

If you have not received the admission form from your doctor, please download and print the file below, and return it as described above.

If you have any difficulties with downloading or completing this form, please contact the admissions office on 03 8807 7122.

On the day of admission please report to Epworth Eastern Main Reception, Level 1. Please bring your health insurance card or details, your Medicare card, Pharmaceuticals entitlements card, Pension Card/Health Care card, current x-rays/scans, medication and personal items.

Hospital accounts must be settled on the day of admission. It is advisable that you contact your health fund or call our Patient Administration Office on 03 8807 7122 to check your level of cover and any likely out of pocket expenses. Your payment can be made in cash, cheque or credit card.

It is important that you are aware of your rights and responsibilities as a patient. A copy of your Rights and Responsibilities will be sent to you prior to admission, or given to you on admission.

Paying your account

Epworth Eastern has agreements with the major health insurance funds. All fees relating to medical practitioner care, pathology and radiology are charged separately. Patients are advised to check with their relevant health fund prior to admission to confirm their level of cover or, alternatively they may telephone the Patient Administration Office at the hospital on 03 8807 7100 or
03 8807 7122.

If you do not have private health cover your Doctor will provide you with an estimate of the anticipated costs. This is then payable on admission. If your treatment costs exceed the estimate provided then these costs will be collected on discharge.

Non-hospital fees will be billed separately and may include: surgeon, anaesthetist, medical services, cardio-pulmonary bypass (perfusion), pathology, pharmacy, radiology/X-Ray, physiotherapy and allied health practitioners (where no arrangement exists between Epworth and your health fund).

Medicare refunds some non-hospital charges and your health fund may also cover a further portion but you will be required to pay the balance. If you have any questions on these charges please check with Medicare and your health fund.
