Pre-Admission Documentation
Your doctor will now have made arrangements for your admission to Epworth Freemasons and provided you with your Pre-Admission Booklet consisting of a Patient Registration Form, Consent Form and Health Assessment.
Admissions are processed through our Admissions Office. It is important that you accurately complete all information on your forms in preparation for your admission and prevent any delays. Please ensure forms arrive at the hospital 5 days prior to your admission.

Confirming Admission Time
Please contact the Day of Surgery admissions area 24 hours prior to your admission to confirm your admission time between 9 - 3pm on 03 9483 3562
Important General Information
If there has been any significant change in your condition (including a cold, cough, fever etc) between the times you last saw your doctor and the day of your admission please make sure your doctor knows as soon as possible.
Smokers should refrain from smoking for at least 24 hours prior to admission.
Please note that the Day of Surgery Area is restricted in space and we ask all patients to be accompanied by only 1 family member/friend.
