Admissions to the hospital are processed through our Bookings Office.
Your doctor will by now have made arrangements for your admission to Epworth Freemasons and you will already have been provided with a Patient Registration and Consent Form. This form should be completed by you and forwarded to the address on the form.
It is important that you accurately complete the health insurance section of your Registration Form. By doing so it may save you a great deal of time and unnecessary worry.
If there has been any significant change in your condition (including a cold, cough, fever etc) between the times you last saw your doctor and the day of your admission please make sure your doctor knows as soon as possible.
If you are a smoker, we advise you not to smoke for at least 24 hours prior to admission.
