Medical Record Access

Patients may access their health information records held by Epworth HealthCare, in accordance with the Victorian Health Records Act 2001 and the Commwealth Privacy Amendment (Private Sector) Act 2000.

You may request either a copy of your medical record, an accurate summary or to view your medical record.

Epworth HealthCare requires payment of fees for access to patient information as per the Health Records Act 2001. Please view or download the file Medical Record Access Fees for further information.

Requests can be made by completing the Medical Record Access Form and returning this with a copy of photo identification to the Privacy Officer, Health Information Services at the Epworth HealthCare campus visited.
