The role of the Epworth GP Liaison Unit is to help strengthen the relationship between Epworth HealthCare and General Practice by:
- Raising awareness in the general practice community of the services and expertise available at Epworth HealthCare.
- Assisting GPs to access hospital services and improving communication between the hospital & GPs with the aim of improving patient care.
- Providing a comprehensive education program for GPs.
The Epworth Healthcare GP Liaison Unit comprises of a GP Coordinator and three GP Liaison staff.
GP Coordinator
Dr Peter Sherwen coordinates a systematic program to improve communication between Epworth HealthCare and referring general practitioners.
The program covers areas including:
- GP involvement in meetings, case conferences and discharge planning by phone or hospital visit
- Monthly interactive educational programs including on-site sessions at several large suburban General Practices. All educational activities are RACGP approved and as such attract RACGP CPD points
- Information about Epworth HealthCare services and admission / referral procedures
- A reliable system to notify GPs of their patients' attendance at ED
A GP Liaison Unit to assist GPs in making rapid referrals to consultants
Facilitation Line
The GP Liaison Unit provides GPs with a successful means of sourcing a specialist consultant. Our GP Liaison staff have a comprehensive knowledge of Epworth HealthCare’s services and a specialist listing of around 500 consultants, in a wide range of specialties. The service saves you time, and facilitates access to a wide range of consultants.
The facilitation line operates Monday to Friday from 9.00am - 5.00pm and GPs can access this service by phoning 03 9426 8504 or 03 9426 6553 (NB: specific patient based enquiries will be directed to Dr Peter Sherwen).
