To help us prepare for your admission we require your admission details form prior to the day of your admission. This form should be sent to our Booking Office at 89 Bridge Road Richmond 3121 or faxed through on 03 9428 7692, as soon as possible.
If you have not received the admission form from your doctor, please download and print the file below, and return it as described above.
If you have any difficulties with downloading or completing this form, please contact the Pre-Admission Centre on 03 9426 6174.
Prior to your admission you should contact your Private Health Fund to confirm that they will cover you for your proposed admission.
You may receive a phonecall from our Patient Service Centre prior to your admission. The purpose of this phonecall is to confirm you details, medical history, advise you of any payments you will need to make on admission and give you an opportunity to ask any other general questions.
Please do not be alarmed if you do not receive a phonecall, in this instance details will be checked with you on admission.
If you have not been provided with a time for your admission, please call the Pre-Admission Centre which is open between 7.00am and 7pm Monday to Friday on 03 9426 6174.
It is important that you are aware of your rights and responsibilities as a patient. A copy of your Rights and Responsibilities will be sent to you prior to admission, or given to you on admission.

Referral System
If you require a referral to a Visiting Medical Practitioner (specialist), please contact their rooms directly. Telephone numbers can be found in the Services Directory which you can download here, or you can use the search boxes on the right hand side of this page.
Urgent Admissions
In case of emergency, patients can be sent by ambulance to the Emergency Department. After hours admission can also be arranged by contacting the on-duty Nursing Co-ordinator on 03 9426 6666. Either a physician preferred by the referring doctor or an on-call specialist will be contacted for the on-going management of the patient.

Elective admissions are arranged with the hospital Booking Office.
Please contact 03 9426 6155 between 8.00am and 5.30pm Monday to Friday.
Paying your account
Epworth HealthCare operates under the auspices of the Uniting Church and is a not for profit organisation with any surplus funds being reinvested in the further development of hospital facilities and patient care.
The hospital has agreements with the major health insurance funds. All fees relating to medical practitioner care (excluding hospital fellows), pathology and radiology are charged separately. Patients are advised to check with their relevant health fund before admission to confirm their level of cover or, alternatively they may telephone the Patient Administration Office at the hospital on 03 9426 6106 or 03 9426 6224.
Non-hospital fees will be billed separately and may include: surgeon, anaesthetist, medical services, cardio-pulmonary bypass (perfusion), pathology, pharmacy, radiology/X-Ray, physiotherapy and allied health practitioners (where no arrangement exists between Epworth and your health fund), and ambulance charges to or from Epworth Richmond.
Medicare refunds some non-hospital charges and your health fund may also cover a further portion but you will be required to pay the balance. If you have any questions on these charges please check with Medicare and your health fund.
Epworth Richmond accepts cash, cheques, EFTPOS and all major credit cards, except Diners and AMEX.
