Community Integration Team

Available at Epworth Rehabilitation Richmond

The Community Integration Team consists of:

• Occupational Therapists
• Speech Pathologists
• Social Worker
• Psychologists
• Physiotherapists

Who provide home and community based rehabilitation for persons with brain injury, and their families.

Once the patient leaves the hospital the team provides this service as part of the ongoing specialist rehabilitation of Epworth.

Referrals from other hospitals or the community may be made for consideration and inclusion to ABI outpatient programmes.

Services Include: 

• Therapy provided in the person’s home, work, school and gym and local community
• Preparing people to return to the workforce or to return to study
• Counselling for individuals and their families
• Assessing and revising the needs for services, supports and equipment
• Assistance to find local community services
• Revision of lifetime needs for clients and their families
