50% of Australians have at least one of eight common chronic conditions (ABS 2015). People who are diagnosed with one or more chronic conditions often have complex health needs, can die prematurely or live a poorer quality of life overall.

GPs, specialists, practice managers, nurses and allied health professionals are invited to our Chronic Diseases Symposium to explore this issue in a series of presentations and workshops.
