Application for Non Specialist Appointment to Practice



Appointment to undertake medical practice at Epworth HealthCare for:
Surgical Assistants or Doctors in Training


Epworth HealthCare has over 2500 doctors practicing in a wide variety of specialties across its campuses. All doctors who practice at Epworth HealthCare are required to undertake a formal appointment process in accordance with the provisions of the Epworth Foundation By-Laws. Please ensure you have read this document prior to applying for appointment to undertake medical practice at Epworth HealthCare.


Before you begin


To submit your application, you will need to have completed the following.


1.     References requirements - Surgical Assistants (only)

If you are only undertaking surgical assisting at Epworth, regardless of your role at any non-Epworth hospital, then you must apply as a Surgical Assistant. The application process includes supplying references as per below. No Surgical Assistant application is accepted as complete until these references are provided, along with other detailed requirements.

Download the blank reference form (on this page) and send it to two referees.  Each referee must:


-          be able to comment on your surgical assisting ability; and
-          at least one must be a surgeon you will assist at Epworth HealthCare; and
-          must not be personally or financially connected to you


 Request for reference surgical assistant - save (if Word document) or scan and save as PDF (if completed by hand) the two completed reference forms, and email through to immediately after completing your online application.  Include both references in one email.  Do not complete the online application form until you are able to undertake this step.  Applications are unable to be processed until both references are received, and will be withdrawn if this information is not promptly provided.


2.  References requirements - Junior Medical Staff (only)

This category only applies if you are either directly employed at Epworth as an intern, HMO. CMO, registrar or fellow OR rotating to Epworth from a public hospital as part of a formal rotation for a specific period.  

Doctors-In-Training (DITs) covered under the above arrangements do not need to attach completed references as these are provided either by your rotating public hospital OR by the department employing you at Epworth.  You must complete all the other detailed requirements.



3.    Ensure you have on-hand the following information or items to upload:


a.     Australian Health Practitioners Regulation Authority (AHPRA) Registration Number

b.     Medicare Prescriber Number

c.     Curriculum Vitae (Word or PDF format)

d.     Medical Indemnity Confirmation Certificate (PDF format)

e.     Referee contact details (address, email and contact number) of the provided references

f.      Relevant undergraduate, postgraduate and specialty fellowship certificates (PDF format) to support your application.

g.   Copy of your current employee Working with Children Check

4.    Note that you will receive an email confirmation after submitting your application online.  You must check that this is received.  If not, then your application has not been successfully lodged and you will need to contact the Junior Medical Workforce Team (on or 03 9426 6226) or Surgical Assisting Team (on or 03 9426 8591) during business hours.  The confirmation email also contains the Epworth Values and Behaviours agreement form. You must sign and return the Values & Behaviours form, along with a copy of your current employee working with children check to be fully submitted.

Please note that submittal of a completed application does not entitle you to work at any Epworth site.  This must be confirmed in writing prior to undertaking any clinical duties.

How long will it take to complete the online application form?


The form will take 15-30 minutes to complete, depending on the volume of information that you provide.


Can I save my application and return to it at a later time?


No - online applications need to be submitted in a single browser session. Applications cannot be saved and returned to at a later time. It is recommended that you have the information outlined above ready before commencing your application.


How will I know if my application has been successfully received?


You will receive a confirmation email when your application has been successfully received.


What happens after I submit my application?


You will receive a confirmation email that contains the Epworth Values and Behaviours agreement form.  You must sign and return that form as per the covering letter letter for your application to be fully submitted. A member of the Medical Workforce Team will be in personal contact if further information is required.​


How long will it take to assess my application?


Applications undergo multiple levels of clinical review before ratification by the relevant Medical Director. Once all required information has been received, an outcome will generally be reached within 15 working days. Note that delays in receiving references are the major source of delay.
