How to request an estimate

To request an estimate for your patient, follow these steps:

  1. Complete the Estimates of Expense Request Form - Surgical or non-surgical
  2. Email the completed form to [email protected]
  3. Our Self-funded Care Team will respond to your request within 1 business day.
  4. A copy of the estimate will then be sent to you for sharing with your patient.
  5. Share our Epworth estimate, along with your services quote, with your patient.
  6. If your patient would like to proceed with their treatment, book your patient at Epworth.

  • Why do I have to submit an estimates request form for each individual patient?

    Estimates are personalised for each patient. Costs can change depending on factors such as the type of procedure, prosthesis requirements, item numbers and length of stay.

  • What if I need to add additional item numbers?

    If you need to add or change anything, such as item numbers, let us know and we will issue an updated estimate.

  • How long is an estimate valid?

    An estimate is valid for 3 months.

  • Can we direct patients to contact Epworth directly?

    We prefer to receive estimate requests through the treating Medical Specialist or Practice to ensure we have the necessary details to provide an accurate estimate.

  • Why has Epworth added prosthesis to the estimate that were not requested or may not be used?

    We base the estimate on procedure history and expected needs. If any items listed in the estimate are not used, the patient will receive a refund.

  • Why does Epworth give estimates based on the worst-case scenario?

    We provide estimates based on the worst-case costs so that a patient can make an informed financial decision on the maximum possible charges.

    For example, if a patient is booked in for a procedure that may vary or is a +/- procedure, we include all possible costs such as extra item numbers, longer stay and prosthesis.

    If any of these costs are not needed, the patient will be refunded. If costs change during their stay, we will adjust the invoice accordingly.

  • Can I share our Epworth estimate breakdown with the patient?

    Yes, we encourage you to provide the full breakdown of costs to your patient to ensure they understand all charges and possible additional charges.
