Hospital in the home means you are treated as an admitted hospital patient, but outside the hospital setting. Epworth doctors, nurses and allied health professionals come to you – providing care in your home or another suitable location.

What services does Epworth at Home offer?

Epworth at Home includes:

• Acute hospital in the home (HITH) - for medical care such as IV therapies, wound care, and pain management
• Rehabilitation in the home (RITH) - tailored rehab programs delivered at home
• Chemotherapy in the home (CITH) - cancer treatment in a familiar environment
• Community Integration - support to help you safely transition home after traumatic injury.

How does it work?

As a patient of Epworth at Home, you’ll receive hospital-standard care without needing to stay in hospital. Your hospital in the home or rehabilitation in the home care team will visit you to provide treatment, monitor your progress, and coordinate any additional support you need.

How can I access Epworth at Home?

If you’re eligible, your treating doctor may offer you hospital in the home, chemotherapy or rehabilitation in the home through Epworth at Home. You can also ask your GP, ward nurse or Epworth doctor if this option is suitable for you.

If it is, we’ll contact you or visit you in hospital to discuss your medical needs, home environment and support network. Everyone involved must agree that hospital in the home is the best option before we proceed.
