Community Integration Services | Hospital in the Home | Epworth at Home

The Epworth at Home - Community Integration Team (CIT) is a group of rehabilitation specialists who help people safely transition from hospital to home and back into their local community after traumatic injury.

Our goal is to support your recovery and independence by delivering personalised care where it matters most - at home, at work, at school, or in your community.

What does the Community Integration Team do?

As part of our rehabilitation in the home model, the Community Integration Team provides:

• Assessment and review of support needs, services and equipment in your home
• Help connecting with local community services
• Counselling for individuals and families
• Support for returning to study or work
• Individual therapy delivered at home, work, school, gym or in the community
• Training to improve everyday living skills
• Ongoing therapy planning for clients with lifetime support needs

We also offer access to centre-based programs that complement individual therapy.

What services does the Community Integration Team provide?

Our team can support you with rehabilitation in the home services and programs, including:

• Dietetic services
• Group programs focused on:
  • Adjustment and coping skills
  • Sport and leisure participation
  • Communication and social skills
• Physical and running skills development
• Support for returning to driving or bike riding

How can I access the Community Integration Team?

The Community Integration Team is available as part of our Epworth at Home service. If you’re already receiving hospital in the home or rehabilitation in the home care, your team can refer you to the Community Integration Team.

If you’re not yet receiving hospital in the home services, speak with your treating doctor, allied health team or discharge planner to find out if you’re eligible.
