To request access to patient medical records for continuum of care or for an upcoming doctors appointment
Health Providers/Organisations
Please send a formal request to our Health Information Services (HIS) Clerical team via e-mail at [email protected] or by fax at (03) 9429 9075 and ensure you follow the guidelines below:
- If requesting via email, please use your professional health organisation email address. If requesting via fax, please use your health organisation's letterhead.
- Include the patient's full name & date of birth (or other three points of ID) and specify which records or information you are after.
Patients
Your GP or specialist is able to request your medical records or information directly from our Health Information Services (HIS) Clerical team via the process outlined above at no cost to you. However if you require this for a claim, personal use, or other purposes, you will have to go through the Medical Record Access process which will incur a fee and can take up to 45 days to process (for more information, please explore the next request section below).
Please note:
Requests are typically actioned within 24 hours, but may take up to 48 hours or longer if you are requesting old records, requested information is for a current ongoing admission, or requires further review by management.

To request access to patient medical records for claims, clinical trial/study, personal use, or other purposes
Law Firms, Insurance Companies, Research Clinics or Other Organisations
Please send a formal request to our Medical Record Access (MRA) team via e-mail at [email protected] and ensure you follow the guidelines below:
- Kindly submit your request on your organisation’s official letterhead and address it to "Epworth HealthCare"
- Include the patient's full name & date of birth (or other three points of ID) and specify which records or information you are after.
- Provide signed consent from the patient and attach this to your e-mail along with your request letter (If not applicable, please refer to page 1 of the Medical Record Access Form for more information).
Patients or Relatives of
Please submit your application to our Medical Record Access (MRA) team and ensure you follow the guidelines below:
- Download the Medical Record Access Form (PDF) and save this file to your computer.
- Complete the form: Once filled, attach the completed form with your copy of your photo identification and return it by e-mail to [email protected]
- Alternatively, the form can be printed, completed by hand, and returned along with a copy of your photo identification by mail to the following address:

In accordance with the Health Records Act 2001, Epworth HealthCare requires payment of fees (outlined in page 2 of the Medical Record Access Form) for access to patient records and have up to 45 days to respond to your request.
Pre-payment is not required for accessing medical records. If provided, please note that no refund will be issued if the request is cancelled.
